Step-by-step process
Determine the Birth Category: Institutional vs. Home Birth
If the child was born in a government or private hospital, the hospital medical officer is legally obligated to submit Form 1 (Birth Report) directly to the local Registrar. Obtain the Hospital Discharge Summary and institutional Birth Slip from the maternity ward. If born at home, the head of household or village Gram Pradhan/ASHA worker must report the birth to the local registrar within 21 days.
Standard Registration Within 21 Days (Zero Penalty)
Apply either online via the national CRS portal (crsorgi.gov.in) or state municipal portals (such as e-NagarSewa, MCD Online, KMC, etc.) or visit your local Municipal Corporation Ward Office / Gram Panchayat. Fill Form 1 with exact spelling of child's name, parents' full names, Aadhaar numbers, and residential address at the time of birth.
Delayed Registration Procedure (After 21 Days)
If 21 days have passed, registration requires statutory sanctions under Section 13: • 21 to 30 Days: Can be registered with payment of a nominal late fee to the Registrar. • 30 Days to 1 Year: Requires written permission from the Tehsildar / Sub-Divisional Magistrate (SDM) along with a late fee. • After 1 Year: Requires a formal Order from an Executive Magistrate (SDM / First Class Magistrate) accompanied by a Notarized Delayed Birth Affidavit on ₹10/₹50 Non-Judicial Stamp Paper, hospital discharge record or immunization card, and verification by local revenue officials.
Document Verification and Inspection
The Registrar verifies the submitted hospital birth slip against hospital register records. In home birth or delayed cases, a local inquiry or verification from the local patwari/parshad is conducted to confirm parentage and date of birth.

Frequently asked questions
Can a child's name be added to the birth certificate later?
Yes. Under Section 14 of the RBD Act, a child can be registered without a first name initially. The parents can submit an application to the Registrar to enter the child's formal name within 12 months free of charge, or up to 15 years with a nominal statutory fee.
How can I obtain a birth certificate if born at home many years ago?
File an application before the Sub-Divisional Magistrate (SDM) / Executive Magistrate of your area. Submit a Non-Judicial Stamp Paper Affidavit, school leaving certificate/matriculation marksheet showing date of birth, parental identity proof, and residential records. Upon magistrate sanction, the local municipal registrar registers the birth and issues the certificate.
